Diabetes & Chronic Disease Management
Professional nursing support for diabetes, hypertension, COPD, and other chronic conditions, including blood glucose monitoring, insulin administration, health education, and regular reviews to promote stability and wellbeing.
Catheter, Continence & Stoma Care
Professional, discreet support including catheter changes, flushing, stoma appliance care, and bowel care. We monitor for infection, promote comfort, and help maintain independence.
Tracheostomy & Respiratory Support
Safe, comfortable and confident support, providing suctioning, tracheostomy care (including cuff and cannula management), and oxygen or nebuliser therapy.
Wound Care
Expert wound care delivered in the comfort of your home, including assessment, dressing changes, and chronic wound management to pressure injury prevention.
Medication Management
Safe and professional medication administration, (including oral, injectable and PR) including assistance with blister packs, injections, medication education, and ongoing monitoring for effectiveness and side effects.
Palliative & End-of-Life Care
Sensitive, dignified nursing care for clients and their families during palliative and end-of-life stages. We focus on symptom management, comfort, and emotional support, working closely with GPs and specialists.
Enteral Feeding (PEG/PEJ/NG)
Comprehensive enteral feeding support, including PEG/PEJ site care, administering feeds and medications, and troubleshooting issues. We ensure safe feeding practices and provide education for clients and families.
Health Assessments & Care Planning
Comprehensive nursing assessments, risk management plans (falls, aspiration, pressure injury), and ongoing review to ensure safe, effective, and person-centred care.
