Disability Services

  • Our disability services include:

    Our supports are tailored to each person’s goals, preferences and needs.

  • Yes. Our team provides personalised in-home supports including personal care, household tasks, meal preparation and daily living assistance to help people live safely and independently in their own home.  

  • Supported Independent Living provides assistance for people who live in shared or individual homes and require support with daily living tasks.

    Our SIL supports focus on independence, choice, safety and creating a stable and supportive living environment.

  • Yes. We regularly review availability across our homes and new opportunities become available throughout the year. We can also seek to tailor solutions according to your needs.

  • ILO supports people to explore flexible living arrangements that suit their lifestyle, preferences and goals.

    This may include living with a host, housemate arrangements or other creative support models that promote independence.

  • Yes. Our operations team works closely with our specialist services team to ensure safe and effective supports are delivered.

    We also collaborate with Behaviour Support Practitioners to implement positive behaviour support strategies that promote safety, dignity and quality of life.

  • Yes. We support participants to build confidence and skills to engage in meaningful community activities such as social outings, recreation, appointments and skill-building opportunities.

  • Yes. We have an accessible vehicle that can transport participants who mobilise using a wheelchair.

    Transport itself is not a primary service we offer but it may be available as part of community access or other supports depending on individual needs and funding.

  • We support both children and adults as long as they have an NDIS funding package.

    Our disability supports typically commence from around 6 years of age, and we currently support participants well into their 70s and 80s. Our supports are tailored to each person’s stage of life and individual goals.

Specialist Services

  • In addition to disability supports, we offer specialist services including:

    • Permanency, Assessment and Support (PAS)

    • Clinical Nursing Services

    • Practice Development

  • PAS services include permanency assessments, carer assessments, complex planning (such as case planning and restoration planning) as well as specialised supports through our Therapeutic Mentoring Program.

    The PAS team offers professional support to assist organisations and systems to make informed decisions about support needs, long-term care arrangements and permanency planning.

  • Yes. Our PAS services support children, young people and adults, providing professional assessment and permanency planning across all life stages.

    Some service types may be aimed at specific age groups or services but the PAS team can offer supports to people of all ages.

  • Yes. Our clinical nursing services support individuals with complex health needs to live safely and confidently in their homes through professional nursing care and health support.

  • Our Practice Development Team offers training, coaching, mentoring and professional supervision.

General questions

  • Yes. All Together Supports is a registered NDIS provider, approved by the NDIS Quality and Safeguards Commission.

    Being a registered provider means we meet strict NDIS Practice Standards, undergo regular audits, and follow the NDIS Code of Conduct to ensure safe, high-quality supports. This provides participants and families with additional confidence in the quality, safety and accountability of the services delivered.  

    Our registration includes a broad range of supports such as personal care, Supported Independent Living, community participation, transport, life skills development, therapeutic supports, behaviour support and community nursing care.

  • We are based in Tamworth, NSW, and provide services across the New England region and surrounding communities.

  • Referrals can be made by participants, families, support coordinators, plan managers, or other professionals. Simply contact our team to discuss your needs and we will guide you through the next steps.

  • Yes. While we are a registered NDIS provider, we support participants who are NDIA-managed, plan-managed, or self-managed.

  • Yes. We run targeted programs designed to support specific groups within our community, including youth-focused supports and our Men’s Circle program.
